Vårdcentralen Valla och Vårdcentralen Ryd. Fördelning är på cirka 70–80 % på Vårdcentralen Valla och cirka 20–30 % på Vårdcentralen Ryd. Du har kontor på respektive vårdcentral. Du samarbetar nära läkare, sjuksköterskor och andra vårdgivare på vårdcentralen och möter en bred variation av patientärenden. Du arbetar folkhälsoinriktat där patientmötet sker enskilt eller i grupp och följer NCP (Nutrition Care Process) där bedömning, diagnos, behandling, uppföljning och utvärdering vid nutritionsrelaterad ohälsa omhändertas evidensbaserat. Vi prioriterar ett gott samarbete i ett tvärprofessionellt team. På vårdcentralen ingår ansvar för framtagande av lokala riktlinjer och internutbildning till medarbetare. Som verksam dietist är du en del av ett dietistnätverk inom regionens primärvårdcentrum med cirka 25 verksamma dietister. Nätverket samordnas av dietist som erbjuder inskolning, handledning. Nätverket bevakar och förmedlar information och nyheter inom region, hälso- och sjukvård samt nutritionsområdet. För ett gott samarbete har dietistorganisationen regelbunden kontakt genom sammankallande professionsmöten som sker digitalt eller fysiskt vid ett tillfälle per månad samt ett nätverksmöte vid 1–2 tillfällen per termin med dietister som är verksamma inom specialistvården. Framtagande av övergripande rutiner, arbetssätt och riktlinjer sker inom dietistorganisationen. Du har möjlighet till professionsspecifik fortbildning. Vi prioriterar patientsäkerhet och arbetsmiljö och har således en regionövergripande prioriteringsplan av patientflöden. Vi har utformat en samarbetsfunktion vid tillfällig frånvaro där du stöttas upp av medarbetare som omhändertar en del av patientflödet under tiden du är borta. Viss del av tjänsten kan föreläggas på distans enligt regionens riktlinjer. Om dig Vi söker dig som är legitimerad dietist och har erfarenhet av din yrkesroll, det är meriterande om du har erfarenhet från arbete i primärvård. Som person är du nyfiken, engagerad och har ett lösningsfokuserat arbetssätt. Arbetet förutsätter att du har god samarbetsförmåga och trivs i att vara en del av ett team. Du ska ha förmåga till självständigt arbete. Det är också viktigt att du har vilja att utveckla verksamheten och kan ta hänsyn till det större perspektivet samt ser till hela verksamhetens bästa. What we’re looking for You don’t need previous nursery experience – we’ll provide the training you need. You’re a passionate and organised chef who takes pride in creating high-quality meals and maintaining excellent kitchen standards.
