Our Care Navigator/ Administrator staff handle all incoming calls, signpost patients to appropriate services and maintain a
welcoming practice environment. You are the first point of contact for patient and provide initial support with queries including
compliments and complaints. You will support clinicians by liaising with patients regarding appointments, referrals,
registrations, general enquires and more. You will also liaise with other healthcare providers to support patient care.
The duties and responsibilities to be undertaken by members of the practice administration team may include any or all of the
items in the following list. Duties may be varied from time to time under the direction of the practice manager, dependent on
current and evolving practice workload and staffing levels

Induction training
On arrival at the practice, all personnel are to complete a practice induction programme.
Learning and development
The effective use of training and development is fundamental in ensuring that all staff are equipped with the appropriate skills,
knowledge, attitude and competences to perform their role. All staff will be required to participate in and complete mandatory
training, as directed by the training coordinator, as well as participating in the practice’s training programme. Staff will also
be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress
their career and, ultimately, enable them to improve processes and service delivery.
Collaborative working
All staff are to recognise the significance of collaborative working. Teamwork is essential in multidisciplinary environments.
Effective communication is essential, and all staff must ensure that they communicate in a manner which enables the sharing of
information in an appropriate manner.
Service delivery
Staff at DMC must adhere to the information contained within practice policies and regional directives, ensuring that protocols
are adhered to at all times. Staff will be given detailed information during the induction process regarding policy and
procedure.
Security
The security of the practice is the responsibility of all personnel. Staff must ensure that they remain vigilant at all times and
report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door
locks with anyone and they are to ensure that restricted areas remain effectively secured.
Professional conduct
At DMC staff are required to dress appropriately for their role.
Leave
All personnel are entitled to take leave. Line managers are to ensure that all of their staff are afforded the opportunity to take
leave each year and should be encouraged to take all of their leave entitlement.
Location
JOB SUMMARY The Care Coordinator (CC) may be required to deal with patients and, if appropriate, their carers, before or after the patient’s consultation with a clinician or other healthcare professional. The CC’s role requires them to be able to work closely with the patient and their clinician or other healthcare professional and understand the roles of, a variety of different people working in the practice and across the PCN. The CC will be involved in coordinating patients’ healthcare and directing them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate You may be given a caseload of identified patients and be required to ensure that their changing or present needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification. What You Bring to the Team * 2–4 years of experience in territory sales, business development, or a similar physician-facing commercial role, with demonstrated success selling into physician practices. * Experience developing new physician relationships and converting prospective practices or providers into customers, partners, or network members. * Experience selling into GI or Cardiology practices strongly preferred; candidates with experience across other physician specialties and a strong track record of physician-facing sales will also be considered. * Demonstrated success with outbound prospecting, pipeline development, and new account acquisition. * Strong relationship-building skills with the confidence and credibility to engage physicians, practice owners, administrators, and healthcare executives. * Results-oriented and motivated by measurable sales goals, pipeline growth, and conversion. * Strong written and verbal communication skills with the ability to clearly articulate a compelling value proposition. * Highly organized and disciplined in managing a territory, pipeline, follow-up activities, and multiple opportunities simultaneously. * Competitive, self-motivated, and comfortable operating with a high degree of ownership in a fast-paced growth environment. * Experience using CRM platforms, LinkedIn Sales Navigator, or other sales prospecting and outreach tools preferred. * Ability to travel within an assigned territory as needed to develop physician relationships, attend meetings, and support partnership development.
